Do I Need A Doctor's Referral To See A Physiotherapist?
No, we do not need a doctor referral to assess you and begin treatment. However, some insurance companies may require a doctor referral in order to issue payments for your treatments.So if you have an extended healthcare plan and are going to claim your costs for the visit through your plan, check to see if a referral is required for reimbursement.
How Can I Make An Appointment?
If you are a new patient there are few options:

1. Please stop by to make your appointment in person. The clinic is situated on the main floor of Dundas Medical Centre, next to pharmacy. Right across to Belleville General Hospital. For location click here.

2. Please use the “new patient intake form” and send it, and we will call you with an appointment at your convenience.

You can find this form “here”. This will save your time on your first appointment, and make easy for the clinician to prepare better with information handy at your first visit.

Please phone the clinic directly to book your appointment at 306-314-5492. (If we are unable to take your call, please leave a detailed message with your contact information and we will be sure to phone you back as soon as possible.)

What Do I Need To Do For My First Appointment?
Please “register online” at least 24 hours before your first appointment. Otherwise make sure you come in 15 min prior to your appointment time to register in the clinic.
What Do I Need To Bring With Me?
1. Personal identification

2. Your extended health insurance information

3. Your referral from your physician if you have one. (Please note that Physical Therapists are first hand practitioners and you do not need a referral to receive treatment)

4. Any written reports from tests that you had done. For example, X-rays, MRI or CT scan. (These reports can be obtained from your local medical clinic or hospital medical records office.)

5. If you are 18 or under, bring your parent or guardian with you. (Unfortunately we will not be able to treat any children 18 and under without first obtaining signed consent from a parent or guardian.)

6 For your own comfort and hygiene, please bring a pair of indoor shoes.

7 Please wear comfortable clothing to your physiotherapy sessions. A pair of shorts is advisable for treatment of lower-body injuries (e.g. Hip, knee, ankle). Examination gowns will be provided as needed.

What Can I Expect From My First Appointment And How Long Are Treatments?
A thorough assessment is done through examination and interview. This is very important to determine an accurate diagnosis and to design a detailed treatment plan to achieve your individual goals.

The first appointment can take up to 90 minutes, so please allow yourself enough time.

Subsequent visits will last between 30-60 minutes depending on the nature of your injury.

How Many Treatments Will I Need?
Many factors, including diagnosis, pre-existing conditions, lifestyle and treatment compliance impact the frequency and duration of therapy. We work closely with both the patient and referring physician to return our patients to their desired level of activity as quickly and safely as possible.
Will My Insurance Cover My Treatment?
Please call us to provide your group health plan number and I.D. number, and our insurance team will contact your insurance provider for details.

If you are using the intake form please fill in your information online and we will call you back.

Most Employee benefit plans / Insurance policies / Extended health plans (like Blue Cross or Great West life etc.) cover physiotherapy and massage therapy services in private clinics. Its a good idea to check with your insurance provider regarding the amount and frequency of coverage you are entitled to.

Can You Bill My Extended Health Insurance Company Directly?
Yes, we do bill to most of the insurance company directly. However, it is advised that when you call, to verify with us if your insurance company is part of the ones we can bill directly.

If not, as a patient, you are required to pay us directly at the time services are rendered or products are received. We will provide you with an official receipt with all the required information to collect your reimbursement from your extended health insurance company.

Is There Child Care In The Facility?
Unfortunately there is no child care available in the facility. Please make arrangements for your child care needs prior to your appointment to ensure your time with your physiotherapist is as productive as possible.
